Transition Years (18-22)

Therapeutic Services

Our mental health services focus on building resilience through strength-based, person-centered treatment. We support individuals—adults, youth, and families—in managing emotions, improving access to quality care, and overcoming social and economic obstacles to mental well-being.

Psychiatric Services

We offer comprehensive psychiatric care, including evaluations, medication management, and a collaborative care approach with therapists and case managers, ensuring holistic, compassionate support for individuals starting at age 6.

Targeted Case Management & Wraparound Services

Targeted Case Management provides coordinated support for individuals and families facing mental health challenges. Wraparound services create individualized care plans, promoting strength and reducing risk through a team-based, collaborative approach.

Steps to Success

Our 12-week social enrichment program is designed to help individuals ages 16 to 21 with Autism Spectrum Disorders build independent living and pre-employment skills. Through a creative and supportive environment, participants gain the tools they need for success.

Employment Options

Our Employment Options program helps adults with disabilities (aged 18 and older) to find and maintain competitive employment in the community. We match the needs of the individual receiving services with the needs of local businesses to find the right match! Our employment consultants help create natural supports on the job so that employment can be long lasting.

Follow Along

Follow Along is an employment retention program for individuals who need long term support in the workplace. Retention Specialists work with participants to help them advocate for themselves and any accommodations they may need, create and/or maintain natural supports, and learn new job tasks to ensure lasting employment.

Youth Employment Services (YES)

The Youth Employment Services (YES) Program helps prepare students with disabilities (aged 14-21) to transition to adulthood by providing opportunities for participation in job exploration counseling, postsecondary educational counseling, self-advocacy training, work readiness training and work-based learning experiences. This is done through 1:1 and group trainings, vocational career camps and community-based work experiences which provide training in a variety of career fields.

Project SEARCH

Project SEARCH is a unique, one-year school to work program for Martin County students (aged 18-21) who have met all graduation requirements. Students participate in employment-related internships in up to three departments within Cleveland Clinic. This program provides real-life work experience, combined with training in employability and independent living skills, to help young adults with developmental disabilities make a successful transition from school to productive adult life.
