Care Coordinator

The Care Coordinator’s core role is to coordinate services across internal teams and service areas to ensure that all programs offered by the organization are seamlessly delivered and aligned to meet client needs from initial contact through discharge. This position supports clients by assessing needs, developing individualized service plans, and facilitating access to appropriate services both within the organization and, when necessary, through community resources.
