Targeted Case Manager

The Targeted Case Manager provides comprehensive care coordination and support services to individuals and families, ensuring access to needed resources and services. This role focuses on assessing client needs, developing individualized service plans, and coordinating care across internal programs and external providers to promote stability, independence, and overall well-being. TCM works collaboratively with internal staff, community partners, and service providers to ensure clients receive comprehensive, client-centered care. TCM promotes the organization’s mission while advocating for client well-being, stability, and independence. This role is also responsible for ensuring services are properly documented, compliant with funding and regulatory requirements, and eligible for billing in accordance with Medicaid and other payer guidelines.
